Understanding the form better

Always remember the following pointers when placing an order:

1. Are all the teeth that shouldn't be moved ( implants, prosthesis etc) marked?
2. If extra space has to be created to give prosthesis later, is that mentioned?
3. If extractions are required.
4. If you are restoring to close open spaces, where they would be preffered?
5. Have all the restorations, veneers, crowns etc been noted.
